Primary Accountability (Job Purpose): 

The Dental Hygienist plays a key role in coordinating and navigating dental care services. Essential Functions: 

  • Plans, organizes, and implements dental outreach events and oral health education activities in the community.  
  • Identifies and enrolls high-risk patients into the oral health services program.  
  • Provides preventive oral health care and screening services both in-house and in community settings.  
  • Maintains and updates tracking systems for patient and community outreach contacts.  
  • Collects and submits monthly reports on patient engagement and community outreach metrics.

Qualifications: 

  • Education: Bachelor’s degree in a health-related field.

Working collaboratively within a multidisciplinary team, the Dental Assistant helps create a positive and comfortable experience for patients in both clinical and school-based settings. 

Essential Functions: 

  • Prepares treatment rooms and patients for dental procedures, ensuring comfort and readiness.  
  • Assists dentists during procedures by sterilizing instruments, suctioning, and handling materials per Georgia law.

The Human Resources Generalist plays a key role in supporting the day-to-day operations of the HR department of Healing Community Health, a Federally Qualified Health Center (FQHC). This position is responsible for performing a wide range of HR duties including recruitment, employee relations, benefits administration, compliance, performance management, and training coordination. The HR Generalist ensures all HR policies and procedures are effectively implemented in accordance with state and federal labor laws, FQHC requirements, and Healing Community Health organizational goals. 

Responsibilities: 

  • Recruitment & Onboarding 
  • Coordinate full-cycle recruitment: job postings, applicant tracking, interviews, offers, and pre-employment screenings. 
  • Ensure credentialing documentation is collected and compliant with FQHC standards.

POSITION: Patient Services Representative

REPORTS TO: Practice Manager

FUNCTIONS: Under general supervision of the Practice Manager, the Front Desk

Support Specialist performs a variety of nonclinical duties in support of the healthcare practice following established methods and procedures involving some independent judgment and latitude in carrying out tasks such as appointment scheduling, patient registration, point-of-service payment collection and other patient inquiry and problem resolution. Provide courteous, unbiased and professional service to patients and assist other clinic staff as required. Reliability and a strong work ethic combined with great communication skills are a must, as well as familiarity with all necessary office equipment, software, and procedures.

RESPONSIBILITIES

  • Greet patients as they arrive at the front desk; determine reason(s) for patient visit to center; direct in accordance with office procedures for check-in, scheduling or walk-in.
  • Checks in patients by verifying and/or updating demographic, insurance, and registration information in accordance with established policies and procedures.
  • Facilitates the scheduling, canceling, and rescheduling of patient appointments and communicates changes to staff as needed.
  • Assist with registration and scheduling appointments at health fairs, outreach programs, and other offsite events.

The Medical Assistant (MA) is to make patients and their needs a primary focus of one’s actions. To develop and sustain productive patient relationships, actively seeks information to understand patient circumstances, problems, expectations, and needs. The MA builds rapport and cooperative relationship with patients, considers how actions or plans will affect patients and responds quickly to meet patient needs and work to resolve any associating problems.

Essential Functions:

  • Collect and record subjective and objective data (i.e., chief complaint, vital signs, height, weight, lab specimens and other department specific measurements).
  • Highly skilled in phlebotomy, lab specimen collections and processing.
  • Electronic Medical Record (EHR) documentation proficiency.
  • Triage and process all messages from patients and co-workers to providers in a timely manner.
  • Maintain clean, organized and stocked exam rooms with supplies and equipment necessary for exams and minor procedures.

Position: Family Medicine Physician

Reports to: Medical Director

Functions: The FM Physician in accordance with established protocols, is primarily responsible for providing medical care for the patients of HEALing Community Health.  In addition, the FM Physician may be directed to perform other duties as directed by the Medical Director within the requirements of contractual agreements and in accordance with licensing, State and Federal funding agencies and other regulatory requirements, and in accordance with the policies and procedures established by the HEALing Community Health Board of Directors.  The person in this position will coordinate and navigate medical care, advocate for systemic health, and motivate and assist people to prevent acute and chronic disease. Additional duties include outreach activities to patients and families, engage patients with motivational interviewing and goal setting. The FM Physician will collaborate and communicate effectively with a multidisciplinary team to effectively coordinate patient care and create a comfortable and exceptional patient experience.

Responsibilities:

  • Provide comprehensive primary healthcare and medical services to patients including ordering laboratory and radiology tests, referring patients to specialist when appropriate, analyzing medical data to diagnose conditions and propose treatment and providing direction to patients regarding lifestyle changes and treatment to improve health.

NPs shall exercise the requisite standard of care, defined as the exercise of at least that degree of skill, care and diligence as would ordinarily be rendered by advanced practice registered nurses, which include assessment of health status, diagnosis, development and implementation of treatment plans, plan of care, and evaluation of patient status.

RESPONSIBILITIES

  • Functions independently to provide health services consisting of health promotion, health screening, management and treatment of acute episodic illness and management and treatment of stable chronic diseases, in addition to, physical for complex acute, critical, and chronically ill patients.
  • Orders and interprets diagnostic and therapeutic tests, lab work, x-rays, medical devices, medical treatment, and diagnostic studies, when necessary, in the management and treatment of such acute illnesses or stable chronic illnesses relative to patient’s age-specific needs.
  • Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities
  • Implements interventions to support the patient to regain or maintain physiologic stability.
  • Monitors the effectiveness of interventions.
  • APRN shall document in writing in each patient’s medical record, electronically or otherwise, those acts performed by (APRN)
  • Collaborates with multidisciplinary team members by making appropriate referrals.
  • Facilitates patient and family decision making by providing educational tools.

Qualifications:

Education

  • Graduate of an accredited Nurse Practitioner Program
  • Current License, Certification, Registration for the State of Georgia
  • Bilingual skills are preferred

Experience

  • Minimum of 3 years of nursing. Ambulatory setting experience preferred.
  • Preferred 1-year experience working independently as a Nurse Practitioners/Advanced Practice Registered Nurse (APRN)

Knowledge, Skills, and Abilities

  • Demonstrate excellent patient experience and organization skills, verbal communication, teamwork, conflict resolution, and the ability to be customer focus, and understand HEAL Community Center customer’s needs
  • Ability to work extended hours, potential half day Saturday hours.
  • Effective computer skills
  • Eligible for hospital privileges
  • Other duties as assigned.

Physical Demands/Working Conditions:   

General office/clinic conditions are pleasant; good, clean working conditions where accident and hazards are negligible; requires short periods of moderate lifting, pushing, or pulling objects up to twenty pounds. Clear diction and acute hearing are necessary for effective communication with the staff and public.

OSHA Classification Category III: tasks that do not involve exposure to blood, body fluids and tissues and the worker can decline to perform tasks which involve perceived risks without retribution.
